CELL ENROLMENT PROCEDURE AND REQUIREMENTS
For Foreign Students on Special Study Permit (SSP)
Effective September 2000
DOCUMENTS REQUIRED
Forms for the following are available at the CELL Office:
• Letter of Request
• Affidavit of Support
• Bank certification in dollars ($800-$900) or equivalent in pesos
* Parent or guarantor stating financial support for the applicant enrollee
(for applicant below 18 yrs. old)
* Copy of passport & visa of parent or guarantor (for foreign national)
* Copy of birth certificate / SSS I.D./ any valid I.D. (for Filipino)
* Self-support stating the applicant enrollee's financial capacity to shoulder related expenses
(for applicant 18 yrs. old & above)
• Photocopy of valid passport and recent entry visa
• Letter of Acceptance
• Guaranty Letter
THE ABOVE DOCUMENTS SHOULD BE SUBMITTED FOR CHECKING AT THE CELL OFFICE AND THE FEE FOR THE ISSUANCE OF THE SPECIAL STUDY PERMIT (Php P 3,730.00) SHOULD BE PAID TO THE BUREAU OF IMMIGRATION, MAGALLANES DRIVE , INTRAMUROS, MANILA .
PROCEDURE FOR APPLYING FOR SPECIAL STUDY PERMIT
Prepare the required documents:
• Letter of Request prepared by the applicant enrollee
• Affidavit of Support prepared by the applicant enrollee or by parent/guarantor and notarized by an attorney
• Bank certification
• Photocopy of valid passport and recent entry visa
• Letter of Acceptance by DLSU-CELL
• Guaranty Letter by DLSU-CELL
Applicant enrollee or parent/guarantor should complete all the necessary documents for checking at the CELL office.
Pay the required fee of Ph P 3,730.00 for issuance of the Special Study Permit to the Bureau of Immigration, Magallanes Drive, Intramuros Manila.
Receive Official Receipt of Payment of SSP.
ENROLMENT PROCEDURE AT CELL
• Present the SSP payment receipt to CELL Office
(for students on tourist visa only).
• Fill out the required Information, Enrolment and Assessment forms.
• Submit one (1x1 or 2x2) photo for file purposes.
• Report for interview.
• Receive Assessment Form and pay the assessed tuition fee to the cashier at the Main Building
(De La Salle University, Taft Avenue , Manila ).
• Receive the official receipt of tuition payment from the cashier.
• Present the official receipt of tuition payment to CELL Office.
• Receive the Identification Card (ID) and Course Cards issued by the CELL Office.
ALL STUDENTS CONCERNED (STUDENTS ON TOURIST VISA) ARE REQUESTED TO PAY THEIR SPECIAL STUDY PERMIT FEES UPON ENROLMENT ONE WEEK BEFORE THE START OF CLASSES TO FACILITATE THE EARLY ISSUANCE OF THEIR PERMIT. ONLY STUDENTS ISSUED THEIR SPECIAL STUDY PERMITS WILL BE ADMITTED TO THEIR CLASSES AT THE START OF THE MONTHLY SESSIONS.
REQUIREMENTS FOR NON-TOURIST VISA STUDENTS
• Photocopy of valid passport and visa(ex. ACR, SIRV, 9(E), 9 (F), etc.)
• Submit 1 pc. of 1X1 or 2X2 picture
